Advice after second failed FET
I just turned 33 and had a second failed medicated FET.
Background: gay/wlw. endometriosis diagnosed via endometrioma on baseline ultrasound. proceeded with stims and banked five euploid 5aa and 5ab embryos. One low level segmental mosaic 5aa.
First transfer proceeded with medicated FET cycle without lupron depot suppression. Was on birth control for about 6 wks followed by about 10 days of daily lupron prior to estrace three times daily and pio + progesterone suppositories twice daily. E2 152 and lining 9.5mm. Initial beta 33, more than doubled to 84 then dropped to 72 and was told to stop meds.
Second transfer, I had to remind my doctor that I had endometriosis that was actually diagnosed by her via ultrasound/ presence of endometrioma and requested lupron and letrozole suppression which she agreed to. Suppressed with lupron and letrozole for 3 months, last shot 3/14. E2 and progesterone were suppressed to undetectable at suppression check before cycle start. Placed on medicated cycle again, with same med schedule except right before the cycle start the clinic messaged me saying they had to move my transfer date up by 5 days and therefore would have me do an extra dose of estrace vaginally for 5 days leading up to lining check/progesterone start “which is totally safe and okay to do.” Well at lining check my e2 was 835 and lining was 15mm. Clinic calls and says everything looks great go ahead and start progesterone. Cut to transfer and everything looks “nice and lush.” No implantation/failed cycle.
We have decided to change clinics as I’ve had to remind this doctor 3 separate times I have endometriosis. It feels like she grouped us into the same sex category and then didn’t make note of the endo she diagnosed. We have now used 2 euploids, leaving 3 euploids and one low level segmental mosaic. What’s really shocking is we had a better outcome / at least implantation with no suppression than we did after 3 months. I’m also reading that that magnitude of e2 elevation can negatively impact receptivity. Also this bleed is not the same as the first failed FET… it’s lighter and not as sticky/weblike. More like a normal period.
Moving forward, should we jump into another cycle (going to request modified natural) or try another retrieval. I’m worried my retrieval numbers will be low given I’m still in suppression window? Any advice welcome.
