[TN] asked a bad question…

Throwaway…I was interviewing and may have accidentally asked an individual a no-no protected question on a phone screen. I quickly corrected course, cut off the response, said sorry and asked the correct form of this question.

They were unqualified for other reasons, were otherwise super professional and I don’t think will be litigious. But what should I do as the hiring manager to document? How likely will this be a problem for me? Should I do any further course correction?
