Hi all,

The title sums it up more or less. I was diagnosed with ADHD back in 2013 with Dr Roy Chad at The Priory, Chelmsford - In true ADHD style, I have lost any documentation for the diagnosis.

I have been asked by my employer for documentation of the diagnosis, and I'm not sure how to find it. I can't find any links to Dr Roy Chad, so I can't track him down.

Does anyone know how I should go about this?
