4 week old, need some advice…

Baby is 4 weeks old and will only sleep at night on my chest (not ideal at all, we’re in survival mode) and will wake every 2-3 hours to breastfeed then go right back to sleep on me. She hates swaddles, hates her crib, has explosive witching hour fits from 8pm-11pm every night. She is a silent reflux baby but we have her on famotidine and are burping her + keeping her upright for 30 minutes after feeds. She does ok sometimes during the day napping if I let her fall asleep on me then transfer her to her bed but never at night. As far as wake windows it’s a little all over the place, sometimes 30-90 minutes awake and maybe a 30 minute nap and other times it’s a few hours of nap and 2 hours awake after.
I’m scared if I keep letting her sleep on my chest she will never be able to sleep independently and that is just not ideal for us.

We do a bath around 7-8 and I nurse her after and then rock her and she usually falls asleep for 20 minutes then wakes up and rages for her witching hours while we try everything to soothe her before finally falling asleep on me around 11pm-2am ish.

Realistically, what can I do/should I change to start to improve this sleep situation? I know she’s still so small and things change quick with babies but I want to try to establish good habits early!

Will this cause a supply issue?

My baby is 4 weeks old and has silent reflux, she wants to latch a lot (presumably for comfort due to throat pain? Maybe cluster feeding?) but she latches, triggers a letdown then gets irritated and unlatches. She does latch and drink when she’s hungry and is gaining weight/has good diaper output so I’m not concerned she’s not getting milk currently. I’m wondering if her latching and triggering a letdown then unlatching multiple times a day will cause any kind of supply issues. Whether it’s undersupply or oversupply, can anyone give me some advice here? Maybe I’m overthinking it all 🤷‍♀️😭😂
